Reminders about Physical Examination and Fee Payment
Students registering for Athletics will have a $150 athletic fee assessed to their Infinite Campus account and must have that fee and ALL OTHER student account fees (current and past due) paid in full AND must have a current physical examination form (SEE LINK BELOW) on file with the PVHS Athletic Office BEFORE the student athlete will be authorized to participate, practice or tryout once the official CHSAA season begins for their sport on 2/24/2024 (Boys Swim/Dive and Boys Volleyball 2/17/2024).
Spring 2025 Sport Athletic fees (the $150 mentioned above) will be assessed by the athletic office to student Infinite Campus accounts upon reciept of your athlete's registration for their sport. Please start monitoring your athlete's account after you've submitted your registration to expedite the payment of all of fees on their account prior to the official CHSAA start to the winter season.
NEW FOR 2024-25 -- The CHSAA Mandatory Physical Form at the link below - this is the ONLYform that will be accepted by the athletic office, athletes turning any other form will not be allowed to try out, practice, compete or participate in any way without the form at the link below.
The new physical form can be uploaded upon registration (preferred), can be faxed to 303-655-8920, emailed directly to the PVHS Athletic Secretary (address below) or athletes can bring them to the athletic office in person. If you upload a photo upon registration or email it to the ahtletic office it has to be a normal sized jpeg photo. Physicals are valid for 1 year from the date of the exam.
